New Patient Information

Once you contact our office and schedule your first appointment, you will receive a new patient packet. Within this packet, you will find a series of forms related to your privacy, current health, and financial information. During your first appointment, we will review your medical and dental history. 

Please assist us by bringing the following information to your consultation appointments:

• Your new patient form and copies of any recent x-rays, if applicable 
• Referral slip from another doctor, if you have one
• A list of medications you are presently taking
• If you have medical or dental insurance, bring the necessary filled out forms. This will save time and allow us to help you process any claims

IMPORTANT: A parent or guardian must accompany all patients under 18 during the consultation visit. Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) If you are required to premedicate before a visit to the dentist, please inform us.
